  1. Right click on "Download the RMA request Form" and select "save target as" to save to your computer.*
  2. Choose the location or path where do you want to save on your computer, then click "ok"
  3. Minimize the browser and double click on the file that you saved on your computer
  4. After double click on the file, Microsoft Excel will open up with the file loaded for printing
  5. The file has total 3 pages for printing.
